Here is an example of what the emergency plans sheet looks like. You can fill it in however you choose to include all of your emergency information in case something were to happen while you were away.
There are also sheets to list you child's extra curricular activities so you and your child's caregivers can keep track of them all.
I had the opportunity to review this book and I think it's great. It really does provide all of the information anyone that is taking care of your children will need. I used to work at a day care about 12 years ago, and having something like this would have been wonderful to keep track of the childrens' information. I don't have very many people that take care of my children because I'm with them most of the time, but I have given sheets to my mother, b/f's father and I also have some filled out in a drawer in the kitchen in case someone ever watches my children here at the house.
